Add New Household

Please email the Center at profiles@csecenter.org and supply the following information for all members of your household that you wish to register:

• Names (First Name, Last Name and Preferred Name if different than First Name)
• Email Address
• Phone Number

Also, for your household, please supply your Address, City, State, Zip Code & Country (if not USA)

On receipt of your information, You will be added into our database.
